Here are the details of the Business License and fees.

Number of employees (don’t count business owner)
0-2 Employees = $50.00
3-5 Employees = $100.00
6+ Employees = $150.00

State License
Beauty Shop Owners, Mobile Home Installers, Plumbers, HVAC, Electricians, General Contractors, and any other business that requires a state license, will need to include a copy of your state license.
Private Employer Affidavit

You will need a Federal Work Authorization User Identification Number if you have more than 10 employees.
